For many youth aging out of the Foster Care system and approaching their 18th birthday, it can be difficult to transition fully into adult independence. The Extended Foster Care program helps youth, ages 18-21 develop a transition plan that fits their skills and strengths to help overcome these challenges.
Extended Foster Care is a statewide program that assigns a youth aging out of the foster care system a Success Coach to meet with them weekly and help build a youth’s strengths and self-advocacy as they enter adulthood.
The goal of the program is to ensure each youth has access to housing, education, medical health services, employment, and permanent healthy connections that can support them in the future.
Each Success Coach works with the youth to develop a transition plan, to help create a vision of what their future may hold. Using the Youth Thrive framework the transition plan will include elements such as:
